Florida DMV Vision Forms
To help make the DMV vision form process as smooth as possible, please review the information below before your visit.
DMV Vision Forms Require an Appointment
Florida DMV “Report of Eye Exam” forms must be completed during an appointment with the doctor after an eye examination.
To make sure everything is accurate and up to date, we’re not able to complete forms that are:
- Dropped off at the office
- Left at the front desk
- Submitted without an appointment
Please Bring Your Form to Your Visit
If you received a notice from the Florida Department of Highway Safety and Motor Vehicles, please bring:
- Your DMV form or official notice
- Your current glasses, if you wear them
- Any updated medication information
Present the form at check-in so our team can prepare it for your appointment.
An Official DMV Notice Is Required
DMV vision forms can only be completed for patients who have received an official request or notice from the Florida DMV.
